SB 6623 - DIGEST

Requires residents of residential habilitation centers to: (1) Be offered placement on the appropriate home and community-based waiver in order to meet the resident's assessed needs, and thereafter to be provided with services that meet his or her needs and maintain his or her welfare, health, and safety; and

(2) Have a plan for discharge to the community.

Requires the department of social and health services to: (1) Establish a procedure for informing residents, and their guardian or legal representatives, of the options for community placement;

(2) Ensure that an individualized transition plan is developed with each resident before moving to the community; and

(3) Establish and implement a written procedure to be used to monitor the adjustment of each resident who leaves a residential habilitation center.
