Every criminal justice agency which authorizes individuals to use its facilities for the purpose of inspecting their criminal history record information must provide an appropriate challenge form and the address of the agency whose record entry is being challenged. Such forms must be substantially equivalent to that set forth in WAC 446-20-450.
