Subject: Suboxone added to expedited prior authorization list for prescription drug program.
Effective Date: October 2003.
Document Description: Effective for the week of September 22, 2003, and after, the Medical Assistance Administration (MAA) will add SuboxoneŠ to the prescription drug program's expedited prior authorization list. This numbered memorandum describes MAA's policy for SuboxoneŠ and include an authorization form.
