Subject: Prescription Drug Medicaid Provider Guide.

Retroactive to dates of service on and after January 1, 2014, Washington apple health (medicaid), administered by the health care authority (HCA) is publishing a revised Prescription Drug Program Medicaid Provider Guide. The revision adds information to clarify how to submit documentation for HCA approval to bill for splitting single dose vials.
