On page 38, at the beginning of line 28, strike "and"
On page 39, line 7, after "surrogate" insert "; and
(k) She must provide to the intended parents copies of all medical
records pertaining to her mental and physical health. The surrogate is
responsible for signing any and all necessary medical release forms to
ensure the records can be disclosed"
EFFECT: Requires a woman acting as a surrogate to release all of her medical records to the intended parents.
