Requires a consumer reporting agency, if certain conditions are met, to place a security freeze for an individual who is: (1) Under the age of sixteen years old at the time a request for the placement of a security freeze is made; or

(2) Incapacitated and for whom a guardian or limited guardian has been appointed.

Requires the issuer of a certified birth certificate to include information prepared by the department of health setting forth the advisability of a security freeze and the process for acquiring a security freeze.
