Requires the state health care authority, the department of social and health services, and the department of health to collaborate to identify goals and benchmarks while also developing individual agency plans to: (1) Reduce the incidence of diabetes in the state;

(2) Improve diabetes care; and

(3) Control complications associated with diabetes.

Provides that this act is null and void if appropriations are not approved.
