    On page 10, after line 13, insert the following:

    "(23) By December 1 of each year, the department must submit an annual report to the appropriate committees of the legislature that includes:

    (a) The reasons why families choose to extend the family assessment response beyond the forty-five day period;

    (b) The barriers that families face in completing the family assessment response within the forty-five day period; and

    (c) The number of families that choose to extend the family assessment response beyond the forty-five day period."

EFFECT:   Requires the Department of Children, Youth, and Families to submit an annual report to the Legislature including:

· The reasons why families choose to extend the family assessment response beyond the 45 day period;

· The barriers that families face in completing the family assessment response within the 45 day period; and

· The number of families that choose to extend the family assessment response beyond the 45 day period.
