On page 6, after line 3, insert the following:

"(4) A person who knowingly provides a false declaration under this section shall be subject to criminal penalties under chapter 9A.72 RCW."

EFFECT: Emphasizes that a person who knowingly provides a false declaration for the purpose of providing informed consent for health care decisions for an incapacitated or incompetent adult or minor is subject to criminal penalties under the perjury statutes.
