On page 2, after line 29, insert the following:
"(4) The provisions of this section only apply to a prospective tenant whose previous landlord provides a statement that the prospective tenant made a good faith effort to cooperate with the landlord during their former tenancy to obtain rent assistance for which the tenant was eligible."
