Provider Bulletin 07-07 Replacement of Linear Analog Hearing Aids.
This provider bulletin became effective September 17, 2007. This bulletin applies to state fund, self-insurance and crime victims' compensation.
To announce a policy about replacing linear analog hearing aids. A linear analog hearing aid issued by the insurer may be replaced with a nonlinear digital or analog hearing aid under the following conditions: The worker returns a linear analog hearing aid to their dispenser or audiologist because the: Hearing aid is inoperable or worker is experiencing an inability to hear, and the insurer has given prior authorization to replace the hearing aid.
The insurer will also pay the associated professional fitting fee (dispensing fee) when the replacement of linear analog with nonlinear digital or analog hearing aid is authorized.

Provider Bulletin 07-08 Insurer Activity Prescription Form (APF).
This provider bulletin becomes effective November 1, 2007. This bulletin applies to state fund, self-insurance and crime victims' compensation. The insurer activity prescription form (APF) is designed to reduce paperwork for healthcare providers by replacing multiple forms with one form. The APF integrates medical information on workers' conditions, restrictions, and prognoses. The goal is to improve communication between healthcare providers, employers, insurers, and workers to aid in return to work.
