Document Description: The department of social and health services (DSHS) health and recovery services administration (HRSA) is updating the Title XIX medicaid state plan via SPA 07-017 to reflect the termination of the contract with Medco for mail order delivery of prescription drugs, as described in Attachment 3.1-A page 32, Attachment 3.1-B page 32, and Attachment 4.19-B page 12.
