INTERPRETIVE OR POLICY STATEMENT
In accordance with RCW 34.05.230(12), following is a list
of policy and interpretive statements issued by the department
of social and health services.
Legal and Administrative Services
Subject: Prior authorization requirements for selected surgical procedures.
Effective Date: April 15, 2012.
Description: Effective for dates of service on and after January 1, 2012, until April 14, 2012, the medicaid program of the health care authority (the agency) will change procedure codes from noncovered to covered without prior authorization (PA):
Effective for dates of service on and after April 15, 2012, the medicaid program of the agency will update procedure codes to be covered with PA through Qualis for clients twenty-one years of age and older:
Effective for dates of service on and after April 15, 2012, the medicaid program of the agency will update codes to be covered with PA through HCA for clients twenty years of age and younger.
