Effective Date: April 18, 2013.

     Purpose: The health care authority intends to submit medicaid state plan amendment (SPA) 13-07 to remove information regarding the chronic care management programs administered by the King County Care Partners, Cowlitz Guidance Association, and United Healthcare. This information is being removed because these programs have been terminated. Services provided by these programs have been since included in the state's healthy options managed care program, as have the categorically needy aged, blind and disabled populations that were served by these programs.
