     Subject: Dental related services medicaid provider guide.

     Effective for dates of service on and after May 1, 2013, unless otherwise specified, the medicaid program of the HCA is republishing the Dental-Related Services Medicaid Provider Guide with coverage table changes and housekeeping changes.
