Notice of Interpretive or Policy Statement
In accordance with RCW 34.05.230(12), following is a list of policy and interpretive statements issued by the health care authority (HCA).

Document Title: Provider Notice #13-46.
Subject: Nondurable Medical Supplies and Equipment (MSE) Medicaid Provider Guide and the Prosthetic and Orthotic Devices Medicaid Provider Guide.
Effective for dates of service on and after July 1, 2013, the medicaid program of the HCA will:

Update the Nondurable Medical Supplies and Equipment (MSE) Medicaid Provider Guide to:

Update the coverage table with information for selected HCPCS codes.
√
Add EPA code 0265 the [to] the expedited prior authorization criteria coding list.

Update the Prosthetic and Orthotic Devices Medicaid Provider Guide coverage table with information for selected HCPCS codes.
•
Update the maximum allowable fees in the following two agency fee schedules:

Prosthetics and orthotics.
√
Medical supplies and equipment.
