In accordance with RCW 34.05.230(12), following is a list of policy and interpretive statements issued by the health care authority (HCA).

Document Title: Provider Notice #13-57.

Subject: Prescription Drug Program Medicaid Provider Guide.

Effective for dates of service on and after September 1, 2013, the medicaid program of HCA will no longer require prior authorization for Rectiv® (nitroglycerin) and will add it to the expedited authorization list.
