In accordance with RCW 34.05.230(12), following is a list of policy and interpretive statements issued by the health care authority (HCA).

Document Title: Provider Notice #13-61.

Subject: Medicaid program of HCA will revise the Nondurable Medical Supplies and Equipment (MSE) Medicaid Provider Guide.

Effective for dates of service on and after October 1, 2013, the medicaid program of HCA will require prior authorization for HCPCS code A4466 and will update the coverage table in the Nondurable Medical Supplies and Equipment (MSE) Medicaid Provider Guide.
