Subject: Sterilization medicaid provider guide.

Effective for dates of service on and after January 1, 2014, the medicaid program of the health care authority (HCA) will update the Sterilization Supplement Medicaid Provider Guide to:

1. Make stylistic changes to improve clarity.

2. Clarify in the "coverage" section that hysteroscopic sterilization must be performed by an agency-approved provider of hysteroscopic sterilization.
