Subject: Ambulatory Surgery Center (ASC) Fee Schedule.

Effective for dates of service on and after January 1, 2014, unless otherwise specified, the medicaid program of the health care authority (HCA) will revise the ASC Fee Schedule incorporating the Year 2014 Current Procedural Terminology (CPT®) codes.
