Title or Subject: Medicaid State Plan Amendment (SPA) 20-0001 Health Home Rate Structure for the Fee-for-Service Delivery System.

Effective Date: When the COVID-19 public health emergency is officially declared over or January 1, 2021, whichever comes last.

Description: The health care authority (HCA), in conjunction with the department of social and health services, aging and long-term care services administration, intends to submit SPA 20-0001 in order continue the rate approved in SPA 20-0032, which was effective July 1, 2020. SPA 21-0001 will take effect when the pandemic is officially declared to be over, and will continue the rate approved in SPA 20-0032. The health home program is a voluntary care coordination model supported by one-on-one, face-to-face visits with eligible high-risk clients who have multiple chronic conditions. The program is available statewide.
