Document Title: DCS Admin Policy 9.30: Witnessing Signatures on Parentage Forms.

Subject: DCS AP 9.30.

Document Description: This DCS administrative policy explains procedures that are in place for DCS staff to witness signatures on department of health percentage [parentage] forms.
