Medicaid Transformation Project (MTP) Housing Transition Navigation Services rate methodology under Health-Related Social Needs (HRSN) program.

The health care authority (HCA) intends to submit a rate methodology for housing transition navigation services provided under MTP 2.0. These services aim to remove barriers to affordable housing, including transition costs and housing deposits to assist with identifying, coordinating, securing, or funding one-time services and modifications necessary to help a person establish a basic household. The navigation services also provide pretenancy and tenancy sustaining services to assist with short- and long-term housing goals and remove barriers to stable housing.

Housing transition navigation services are expected to increase the annual aggregate expenditures/reimbursement/payment for housing providers by $18 million under MTP 2.0. Providers will be able to access MTP funds to provide new housing services to participants in the foundational community supports (FCS) program not covered by existing support services.

HCA welcomes interested parties to submit comments and concerns about the rate methodology and/or the effects no later than Friday, August 30, 2024.
