     On page 2, line 22, after "unless", strike all material to and including "services." on line 29 and insert "at or prior to the time of the referral a disclosure is made of the existence of such an arrangement to the person being referred."

EFFECT: Deletes provisions requiring disclosure of charge or range of charges for services and removes disclosure that a person is not required to use a particular service provider.
