      Establishes an alcohol server permit to be known as a class 12 permit.

      Establishes procedures for issuance of the permit.

      Requires the board to regulate a required alcohol server education program.

      Requires proof of completion of an approved alcohol server training program for the issuance of a permit after July 1, 1993.

      Prescribes criminal penalties for violations of the act.
