    On page 9, line 13 of the amendment, after "procedures;" strike "and"

    On page 9, line 19 of the amendment, after "individuals" insert "; and

    (3) Conduct annual enrollee satisfaction surveys and provide survey results to their enrollees.  The department of health shall set the form of such surveys in rule based on the recommendations of the health care policy board in consultation with health carriers"
