     On page 2, beginning on line 20, after Ato@ strike all material through Anewborn@ on line 21 and insert Adisclose his or her identity, the identity of the other parent and any known medical history relevant to the child@

     On page 2, beginning on line 22, strike all of subsection (b)

     Renumber the remaining subsections accordingly

EFFECT: Requires the transferring parent to provide his or her name, the name of the other parent, and any relevant medical information at the time of the transfer.  Removes the requirement that the hospital provide referral information about adoption, counseling, and other services.
