On page 2, line 33 of the amendment, after "stalking;" strike "and"

On page 2, line 34 of the amendment, after "occurred" insert "; (iii) the location where the act or acts occurred; (iv) a brief description of the act or acts of domestic violence, sexual assault, or stalking; and (v) the name of the alleged perpetrator or perpetrators of the act or acts"

On page 3, beginning on line 6 of the amendment, strike all of lines 6 and 7 and insert the following:

         On page 4, line 13 of the amendment, after "section." insert the following:

          "(4) Within 90 days of providing a record of a report of an act of domestic violence, sexual assault, or stalking to a victim or a household member under this section, the qualified third party must file a copy of the record of a report with the local law enforcement agency."

EFFECT: Requires the written record of a report of abuse to a qualified third party to include the following additional information: (1) A brief description of the act of domestic violence, sexual assault or stalking; (2) the location where the act occurred; and (3) the name of the perpetrator of the act. Modifies the sample report to include this additional information.

Requires a qualified third party to file a copy of the record of a report of domestic violence, sexual assault or stalking with the local law enforcement agency within 90 days of providing the record of a report to a victim or household member.
