On page 2, line 9 of the amendment, after "member" strike "is a victim" and insert "was a victim of an act that constitutes a crime"

On page 2, line 33 of the amendment, after "acts" insert "that constitute a crime"

On page 3, line 11 of the amendment, after "individual" strike all material through "victim" and insert "was the victim of an act that constitutes a crime"

EFFECT: Specifically states that the act of domestic violence, sexual assault or stalking that is the basis of the report and termination of the tenancy must be an act that constitutes a crime of domestic violence, sexual assault or stalking.

Amends the sample form to require the qualified third party to certify that the individual was the victim of an act that constitutes a crime of domestic violence, sexual assault or stalking. (Underlying amendment requires certification that the victim informed the third party of the person's status as a victim.)
