Provides that application forms for initial or renewal licensure, certification, or registration to a health care profession must contain a provision asking the applicant whether or not he or she grants permission to the department to forward his or her name, profession, and contact information to the military department as a possible volunteer resource to be contacted in the event of a bioterrorist incident, natural disaster, or other emergency or disaster, as defined in RCW 38.52.010, that requires the services of health care providers.
