Provides that, upon receipt of information to be incorporated into the registry data base, the contractor or subcontractor responsible for entering the information shall mail to the address provided for each cancer patient printed information describing the purpose of the cancer registry program; the collection, use, distribution, and confidentiality of the information; and other information that the department deems necessary.

Takes effect July 1, 2005.
