On page 4, beginning on line 12, after "drug," strike all material through "subsection." on line 23, and insert "the pharmacy benefits manager shall disclose to the covered entity the cost of both drugs and any benefit or payment directly or indirectly accruing to the pharmacy benefits manager as a result of the substitution. No pharmacy benefits manager is required to disclose the information required in this subsection to the covered entity under any of the following circumstances:
     (i) The drug substitution is initiated for patient safety reasons;
     (ii) The currently prescribed drug is no longer available in the market; or
     (iii) The substitution is required for coverage reasons in which the prescribed drug is not covered under the covered person's formulary or the covered entity's prescription drug benefit plan."
