"NEW SECTION.  Sec. 7   (1) Every retainer health care practice shall annually, before the first day of March, file with the commissioner and the health care committees of the house of representatives and senate a statement showing its financial condition as of the last day of the preceding calendar year. The statement shall also include the retainer health care practice's number of retainer subscribers, number of medicaid and medicare enrollees, and amount of charity care provided. The statement shall be in such form as is furnished or prescribed by the commissioner. The commissioner may for good reason allow a reasonable extension of the time within which such annual statement shall be filed.
     (2) The commissioner may suspend or revoke the retainer health care practice's authorization to operate under this chapter if the retainer health care practice fails to file its annual statement or pay the fees when due or during any extension of time therefor which the commissioner, for good cause, may grant."
