    On page 3, line 6, strike "consort" and insert "concert"

    On page 3, line 14, strike "return to" and insert "initiating"

EFFECT:  Makes a technical change to the more commonly used "concert" rather than its homophone.  Provides that the information sheet must be signed and returned before a youth athlete begins practice or competition.
