Provides services and support arrangements for the transition of persons with developmental disabilities from an institutional setting to a community setting.

Requires the department of social and health services to: (1) Submit an implementation plan to the legislature that provides costs and timelines for establishing living arrangements for residents transitioning out of residential habilitation centers; and

(2) Implement the plan once it is approved by the legislature.
