Provides that certain group or individual plan contracts must: (1) Provide coverage for certain preventive health services without imposing cost sharing; and

(2) Cover a minimum of two courses of treatment in a twelve-month period for certain tobacco cessation treatments.

Provides a contingent expiration date.
