Requires the following licensees to provide information on primary place of practice, as requested by the secretary of the department of health, at the time of license renewal: Dental hygienists, dentists, midwives, pharmacists, physicians, physician assistants, registered nurses, advanced registered nurse practitioners, nurse practitioners, licensed practical nurses, mental health counselors, marriage and family therapists, social workers, psychologists, and chemical dependency professionals.
