On page 5, after line 19, insert the following:

"(c) The attending qualified medical provider and the consulting qualified medical provider selected by the qualified patient may not have a supervisory relationship with each other."

EFFECT: Requires that the qualified patient choose an attending qualified medical provider and a consulting qualified medical provider who do not have a supervisory relationship with each other.
