(1) To be considered eligible with cerebral palsy, you must have a diagnosis of cerebral palsy or similar cerebral damage which causes full or partial limb paralysis, with evidence of onset before age 18.
(2) DDA accepts a diagnosis from:
(a) A licensed physician;
(b) A licensed naturopathic physician; or
(c) A physician assistant or advanced registered nurse practitioner (ARNP) associated with a neurological practice.
