(1) To be considered for eligibility with another neurological or other condition similar to intellectual disability you must:
(a) Have a diagnosis of a neurological or chromosomal disorder that:
(i) Originated before age 18;
(ii) Is known by reputable authorities to cause intellectual and adaptive skill deficits;
(iii) Is expected to continue indefinitely without improvement;
(iv) Is other than intellectual disability, autism, cerebral palsy, or epilepsy; and
(v) Is not attributable to nor is itself a mental illness, or emotional, social, or behavior disorder; or
(b) Be receiving fee-for-service medically intensive children program (MICP) services under WAC
182-551-3000, and have been continuously eligible for DDA due solely to your MICP eligibility since before August 13, 2018.
(2) You must have been diagnosed by:
(a) A licensed physician;
(b) A licensed naturopathic physician;
(c) Geneticist; or
(d) One of the following professionals associated with a neurological clinic or genetic testing center:
(i) An ARNP; or
(ii) A physician assistant.
