(1) DDD uses your ADL support needs score and the following table to determine your ADL support needs level for the SIS assessment if you are age sixteen or older:

0 or 1 | None | 0 |
(2) If you have a "Type of support" score of four for any of the questions listed in WAC
388-828-5460, your ADL support needs level for the SIS assessment is determined to be high.
