(1) The person-centered service plan is the primary tool DDA uses to determine and document your needs and to identify the services to meet those needs.
(2) Your person-centered service plan must include:
(a) Your identified health and welfare needs;
(b) Both paid and unpaid services and supports approved to meet your identified health and welfare needs as identified in WAC
388-828-8040 and
388-828-8060; and
(c) How often you will receive each waiver service, how long you will need it, and who will provide it.
(3) For any person-centered service plan, you or your legal representative must sign the plan indicating your agreement to the receipt of services.
(4) You may choose any qualified provider for the service, who meets all of the following:
(a) Is able to meet your needs within the scope of their contract, licensure, and certification;
(b) Is reasonably available;
(c) Meets provider qualifications in chapters
388-845 and
388-825 WAC for contracting; and
(d) Agrees to provide the service at department rates.
