(1) An applicant for vocational rehabilitation services shall be interviewed personally by a vocational rehabilitation professional or paraprofessional within ten working days upon receipt of an application by the department.
(2) The interviewer shall:
(a) Explain to the applicant the nature and operation of the vocational rehabilitation program as it relates to the applicant;
(b) Specifically inform the applicant of the right to appeal any eligibility decision made by the department on his or her behalf through mediation and fair hearing in accordance with WAC
67-25-600;
(c) Inform the applicant of his or her right of confidentiality of information possessed by the department and conditions for its release in accordance with WAC
67-25-051;
(d) Inform the applicant of the vocational rehabilitation concept of informed choice, and how it relates to the individual's rehabilitation process in accordance with WAC
67-25-061;
(e) Provide to the applicant a description of client assistance program services; and
(f) Obtain information from the applicant necessary to determine his or her eligibility for vocational rehabilitation services in accordance with WAC
67-25-125 and
67-25-130.
