(1) To obtain a waiver, a waiver form must be submitted by the district to the superintendent of public instruction. The principal of the school or designee shall review the information and transmit through the district office the wavier [waiver ] form and a copy of the student's assessment score report or transcript, if listing the state test scores, to the superintendent of public instruction via the graduation alternatives online application.
(2) Upon approval: The student's transcript shall indicate that the applicable certificate was waived.
