(1) Initial notification. Each community solar company must notify the commission orally or by email of any accident that results in death or serious injury to any person occurring in connection with a company's community solar project or through contact with its facilities no later than the second business day following the company's discovery of the accident.
(2) Follow-up report. The community solar company must submit a follow-up written report of the accident to the commission within fifteen business days of the initial notification. The report must include, at a minimum:
(a) The time and place of the accident;
(b) A brief description of how the accident occurred;
(c) Whether the accident resulted in a fatality;
(d) The name and address of the persons injured; and
(e) A brief description of the company's response to the accident including, but not limited to, any medical treatment that was provided to the injured persons.
