аааа Effective Date: January 1, 2012.

аааа Description: Effective for dates of service on and after January 1, 2012, the medicaid program of the health care authority intends to:

Х аааа Begin using the year 2012 current procedural terminology (CPTж) and healthcare common procedural coding system (HCPCS) level II and modifiers; and

Х аааа Update the blood bank services fee schedule and coverage table to reflect changes within this memo.
