Document Title: DCS Administrative Policy 9.30: Witnessing Signatures on Parentage Forms.

Subject: Witnessing signatures on parentage forms.

Effective Date: January 1, 2019.

Document Description: This DCS administrative policy explains procedures that are in place for DCS staff to witness signatures on department of health parentage forms.
