On page 3, line 3, after "(4)" insert "(a) Education agencies may request student social security numbers for research purposes. Each local school board shall adopt a policy on the disclosure of student social security numbers for research conducted by an education agency. The policy shall include written notification to the student age eighteen or older or to the parent or legal guardian of a student under age eighteen. The notification shall include:
(i) An explanation that disclosure of the social security number is voluntary and that no benefit is contingent upon disclosure;
(ii) A statement that the signed consent by the student age eighteen or older or the parent of a student under age eighteen is required for the disclosure of the social security number;
(iii) A statement that the social security number shall be used for research purposes only;
(iv) A statement that the research shall be conducted by an education agency or the agent of an education agency;
(v) A statement that research and data management personnel only may have access to the social security number; and
(vi) A statement that research and data management personnel may not disseminate individually identifiable information to other persons.
(b) Written consent to the disclosure of the social security number by the student age eighteen or older or the parent or legal guardian of a student under age eighteen following the notification required in subsection (3)(a) of this section shall be considered as meeting the information requirements of subsection (5)(a) of this section.
