   On page 2, after line 2, insert the following:

   "Each member state shall require only the following information in order for a person to register to vote: name; residential address; a valid driver's license or identification card for the state in which the person is registering to vote or the last four digits of his or her Social Security number; date of birth; a signature attesting to the truth of the information provided on the application; and an indication on the application that he or she is a United States citizen."

EFFECT: Requires each member state requires the same information on a voter registration application: name; residential address; a valid driver's license or identification card or the last four digits of his or her Social Security number; date of birth; a signature attesting to the truth of the information provided on the application; and an indication that he or she is a U.S. citizen.
