   On page 2, after line 2, insert the following:

   "Each member state shall require only the following information in order for a person to register to vote: the address of the last former registration of the applicant as a voter in the state; the applicant's full name; the applicant's date of birth; the address of the applicant's residence for the purposes of voting; the mailing address of the applicant if different than his or her residential address; the gender of the applicant; the applicant's state driver's license number or identification card number or the last four digits of the applicant's Social Security number if he or she does not have a driver's license or identification card; an indication if the applicant is a member of the armed forces, national guard, reserves or an overseas voter; an indication that the applicant is over eighteen years of age; and an oath that the applicant must sign attesting to the veracity of the information he or she provided on the application."

EFFECT: Requires each member state requires the same information on a voter registration application.
